Create New User

Create a user name. Spaces are allowed. Punctuation is not allowed except for periods, hyphens and underscores.

A valid e-mail address is required to receive notifications. All e-mails from this system will be sent to this address. The e-mail address will be used by MAC if you wish to receive a new password or wish to receive certain news or notifications by e-mail; however note that it may also be available to the public upon request. If at any time you wish to unsubscribe, log into this site and there you will be able to manage/remove your subscriptions.

Create a password and enter it in both fields. Your password may be private data under Minnesota law. You are not required by law to provide a password, but if you do not provide it MAC will be unable to give you access to your profile. The private data may be shared to the extent permitted or required by law or court order. In contrast, your name and contact information, including your email address, may be available to the public upon request.
